The central theoretical disagreement regarding hypnosis is known as the "state versus nonstate" debate. When Braid introduced the concept of hypnotism, he equivocated over the nature of the "state", sometimes describing it as a specific sleep-like neurological state comparable to animal hibernation or yogic meditation, while at other times he emphasised that hypnotism encompasses a number of different stages or states that are an extension of ordinary psychological and physiological processes. Overall, Braid appears to have moved from a more "special state" understanding of hypnotism toward a more complex "nonstate" orientation.

Depending on the purpose of the hypnotherapy (i.e., smoking cessation, weight loss, improvement in public speaking, or addressing some deep emotional turmoil), follow-up may be advisable. When trying to eradicate unwanted habits, it is good practice to revisit the therapist, based upon a date prearranged between the therapist and the patient, to report progress and, if necessary, to obtain secondary hypnotherapy to reinforce progress made.
Hypnosis is normally preceded by a "hypnotic induction" technique. Traditionally, this was interpreted as a method of putting the subject into a "hypnotic trance"; however, subsequent "nonstate" theorists have viewed it differently, seeing it as a means of heightening client expectation, defining their role, focusing attention, etc. There are several different induction techniques. One of the most influential methods was Braid's "eye-fixation" technique, also known as "Braidism". Many variations of the eye-fixation approach exist, including the induction used in the Stanford Hypnotic Susceptibility Scale (SHSS), the most widely used research tool in the field of hypnotism. Braid's original description of his induction is as follows:
Before long, hypnotism started finding its way into the world of modern medicine. The use of hypnotism in the medical field was made popular by surgeons and physicians like Elliotson and James Esdaille and researchers like James Braid who helped to reveal the biological and physical benefits of hypnotism. According to his writings, Braid began to hear reports concerning various Oriental meditative practices soon after the release of his first publication on hypnotism, Neurypnology (1843). He first discussed some of these oriental practices in a series of articles entitled Magic, Mesmerism, Hypnotism, etc., Historically & Physiologically Considered. He drew analogies between his own practice of hypnotism and various forms of Hindu yoga meditation and other ancient spiritual practices, especially those involving voluntary burial and apparent human hibernation. Braid's interest in these practices stems from his studies of the Dabistān-i Mazāhib, the "School of Religions", an ancient Persian text describing a wide variety of Oriental religious rituals, beliefs, and practices.

Émile Coué (1857–1926) assisted Ambroise-Auguste Liébeault for around two years at Nancy. After practising for several months employing the "hypnosis" of Liébeault and Bernheim's Nancy School, he abandoned their approach altogether. Later, Coué developed a new approach (c.1901) based on Braid-style "hypnotism", direct hypnotic suggestion, and ego-strengthening which eventually became known as La méthode Coué. According to Charles Baudouin, Coué founded what became known as the New Nancy School, a loose collaboration of practitioners who taught and promoted his views. Coué's method did not emphasise "sleep" or deep relaxation, but instead focused upon autosuggestion involving a specific series of suggestion tests. Although Coué argued that he was no longer using hypnosis, followers such as Charles Baudouin viewed his approach as a form of light self-hypnosis. Coué's method became a renowned self-help and psychotherapy technique, which contrasted with psychoanalysis and prefigured self-hypnosis and cognitive therapy.
More than 200 years later, research in neuroscience is confirming at least parts of Mesmer’s outlandish theory. No, there is not magnetic fluid coursing through our bodies. But the power of mere suggestion — of imagination, as Franklin phrased it — is a more effective treatment than many modern skeptics might expect, causing real, measurable changes in the body and brain. Hypnotism has been shown to be an effective treatment for psychological problems, like phobias and eating disorders, but the practice also helps people with physical problems, including pain — both acute and chronic — and some gastrointestinal diseases. Physicians and psychologists have observed this with their own eyes for decades; now, many of them say that brain-imaging studies (not to mention the deep respect people tend to have for all things prefixed by “neuro”) are helping them finally prove their point.
Published, controlled studies of the use of hypnosis to cure warts are confined to using direct suggestion in hypnosis (DSIH), with cure rates of 27% to 55%. Prepubertal children respond to DSIH almost without exception, but adults often do not. Clinically, many adults who fail to respond to DSIH will heal with individual hypnoanalytic techniques that cannot be tested against controls. By using hypnoanalysis on those who failed to respond to DSIH, 33 of 41 (80%) consecutive patients were cured, two were lost to follow-up, and six did not respond to treatment. Self-hypnosis was not used. Several illustrative cases are presented.

Meditation practice focuses on stilling or emptying the mind. Typically, meditators concentrate on their breath or a sound (mantra) they repeat to themselves. They may, alternatively, attempt to reach a state of “detached observation,” in which they are aware of their environment but do not become involved in thinking about it. In meditation, the body remains alert and in an upright position. In addition to formal sitting meditation, patients can be taught mindfulness meditation, which involves bringing a sense of awareness and focus to their involvement in everyday activities.
Although most practitioners receive their training in hypnotherapy or relaxation as a part of their academic training, the American Society of Clinical Hypnosis and the Society for Clinical and Experimental Hypnosis maintain training programs as well as a registry of practitioners (see previous box). Training in teaching relaxation techniques is provided through various routes from self-teaching and apprenticeships to a number of short courses. Many yoga centers also teach relaxation and offer courses to train yoga teachers.teachers.

The practice of many relaxation techniques is poorly regulated, and standards of practice and training are variable. This situation is unsatisfactory, but given that many relaxation techniques are relatively benign, the problem with this variation in standards is more in ensuring effective treatment and good professional conduct than in avoiding adverse effects. By selecting a license mental health professional (psychologist or social worker), patients are more likely to receive treatment from individuals who are well trained in the appropriate use of behavioral techniques.
There are varying theories throughout both the medical and psychological arenas as to how the process of hypnosis works. Some experts believe that people who practice hypnosis effectively are predisposed to this therapy or have developed enhanced cognitive and interpersonal abilities that allow them to respond accordingly to hypnotic cues and conditions. Recent studies have shown that this form of communication actually alters elements of a person’s neurological and physiological mechanisms.
Hypnotherapy is guided hypnosis, or a trance-like state of focus and concentration achieved with the help of a clinical hypnotherapist. This trance-like state is similar to being completely absorbed in a book, movie, music, or even one's own thoughts or meditations. In this state, clients can turn their attention completely inward to find and utilize the natural resources deep within themselves that can help them make changes or regain control in certain areas of their life.
